My tire has been completely bald/smooth for a few months now. I haven't
experienced any problems yet, but should I worry? What effect does the bald tire
have on the inner tube?

If you let the tire bald to the extent that the intertube shows through a sharp
object is bound to penetrate the tube (especially if you can almost see the
tube); if you continue to rotate your tire (if it is balding in one area from
spinning, idling or sharp turns) this will conserve it.
