Freewheel (Drift Trike) hub failures.

Now that Freewheel's getting serious and people are breaking hubs regularly, we need some new ones (especially now the Nimbus ones are out of production). I'm in discussions with Onyx about this (square taper and potentially ISIS).

To get a good idea of what's failing on the freewheel hubs we've used so far (Nimbus and others) and work out what we need to improve if we get some manufactured, can we get a list of hub failures?
Please only post your own hub failures (unless you're posting on behalf of someone who doesn't use this forum/Facebook).
